Service launches to connect unattached patients with primary care across West Parry Sound

The West Parry Sound Ontario Health Team is launching CareBridge, a new partnership designed to provide interim health services and help connect patients in the region with permanent primary care providers.

The service is being provided in partnership with the West Parry Sound Health Centre Rural Nurse Practitioner Clinic and Parry Sound Family Health Team.

“CareBridge represents a collaborative and practical solution to bridge the gap in primary care services,” said Katelyn Beattie, nurse practitioner and clinical manager of the CareBridge Clinic. “It ensures patients without a primary care provider have a place to turn for timely, non-emergency care and provides a pathway to permanent primary care.”

Local health officials said the initiative responds to the province’s directive for Ontario Health Teams to facilitate patient attachment through innovative, community-based models.

CareBridge will support individuals without a family doctor or nurse practitioner anywhere in the province, ensuring access to medical care and a pathway to long-term primary care. It will serve patients on an appointment-only basis.
