Patient and social services navigator joins care team at West Parry Sound Health Centre

The Parry Sound hospital has added a patient and social services navigator to its staff.

Officials say longtime employee Bobbi-Jo Henry, transitioned into the role on March 5.

Henry will provide support for patients, families and caregivers, and work to strengthen interconnectivity between the health centre and other health and social service partners in the region.

“We are confident that this role will create positive impacts within our communities,” said Donald Sanderson, chief executive officer for the hospital. “While Bobbi-Jo is located within the health centre, we see this role as a community resource available to our partners and citizens.”

Officials say they understand the health care system can be complex for patients and families to navigate, and each patient has unique care needs. Henry will help ease the burden of navigating the entire health and social services system by providing effective and efficient guidance to those in need.

“Bobbi-Jo will be supporting patients and families within the health centre. However, much of her work will be focused on following up and providing support to patients after they have been discharged,” said Victoria Langley, director of nursing and clinical services.

“She will ensure that they continue to have access to the support they need and will be able to connect individuals with services such as counselling, mental health supports, disability programs, exercise programs, and (other) community services.
