Patients needed in the Parry Sound District for remote patient care demonstration project

Parry Sound District residents living with chronic heart failure or chronic obstructive pulmonary disease can be a part of a demonstration project.  The Town of Parry Sound along with the West Parry Sound Health Centre and Future Health Services Inc. are working to deliver the project.  It uses specialized in-home medical technology to monitor patients and connect them with a team of medical professionals.  The technology monitors a patient’s vital signs daily  and if warning signs show up, a local paramedic is automatically notified and checks-in on the patient before symptoms get worse.  Patients selected for the study will be visited by a local paramedic who will set up the equipment in their home.  Each patient will participate in the program for three to six months while the program will runs for 18 months in total.  If you or a loved one is interested, call 1-866-997-9928.
